FARMAME is a gazetteer of medicines with usual, maximum and minimum doses defined on a clinical scientific basis and agreed with healthcare professionals. Using the gazetteer and prescription data from prescription XXI (and private prescriptions), an algorithm has been developed that provides the pharmacist and patients with information on: Adherence to therapy and persistence; Stopping inappropriate patterns of prescribed medication; Correct mode of medication administration. When a patient comes to the pharmacy with the electronic prescription (prescription XXI) the system analyses the active and non-active medicines and allows to alert the pharmacist when a patient has an anomalous behaviour with respect to the medicines he/she takes. The company is also implementing an application (App) dedicated to the patient where he/she can find his/her pharmacotherapeutic history and information about his/her pharmacotherapeutic treatment.
